Your Health
It makes sense being in control of your health, self monitoring and managing it. The surgery encourages patient’s to take the lead with regards to their health.
If you already suffer from Asthma the surgery would like to know how stable it is and whether it affects your sleep, exercise, activities. Please answer some short questions so that we can assess your Asthma better.
The surgery may have provided you with a peak flow and inhaler. Follow the instructions and provide peak flows to check if you have asthma or not

If you suffer from Hypertension or advised to monitor your blood pressure at home why not notify the surgery of your blood pressure once a year to make sure it is well controlled.
